Finding
Revision 1: insert plate extended to 22 inches after the second UT pass found the thin band running further toward the 4 o'clock position.

Findings
A follow-up UT pass after grit blasting extended the affected band to roughly 22 in. Low reading is now 0.204 in at grid B5.

Recommended options
- Extend the insert plate to cover the full 22 in band rather than patching twice.
Justification
The original scope stopped short of the thinned area, which would have left sub-minimum wall in service next to a new weld.
