FINDINGS:
During the 2026 Olefins Unit Turnaround, visual inspection of D-215 (Spent Caustic Surge Drum) revealed corrosion that exceeded the acceptance criteria per API 510 section 7 / API 579-1 Part 4. Refer to the information below for specific dimensions of the aforementioned defects.
A follow-up UT pass after grit blasting extended the affected band to roughly 22 in. Low reading is now 0.204 in at grid B5.
RECOMMENDATIONS:
- Option 1: Extend the insert plate to cover the full 22 in band rather than patching twice.
JUSTIFICATION(S):
The original scope stopped short of the thinned area, which would have left sub-minimum wall in service next to a new weld.
